Description

PROPERTIES:
DIAPHAGE 1000 tablets contain metformin HCI, an oral antihyperglycemic agent used in the management of non-insulin dependent diabetes mellitus (NIDDM, type 2 diabetes). DIAPHAGE 1000 improves glucose tolerance in NIDDM patients, lowering both basal and postprandial plasma glucose. DIAPHAGE 1000 decreases hepatic glucose production, reduces intestinal absorption of glucose and improves insulin sensitivity (increases peripheral glucose uptake and utilization). Unlike sulphonylureas, DIAPHAGE 1000 does not produce hypoglycemia in either diabetic or nondiabetic subjects and does not cause hyperinsulinemia. Metformin-treated patients show significant improvement in all parameters of glycemic control, stabilization or decrease in body weight and a tendency to impovement in the lipid profile.

INDICATIONS:
DIAPHAGE 1000 as monotherapy is indicated as an adjunct to diet to lower blood glucose in patients with type 2 diabetes whose hyperglycemia can not be satisfactorily managed on diet alone. DIAPHAGE 1000 may be used concomitantly with a sulfonylurea or insulin to improve glycemic control.

DOSAGE:
The usual starting dose is 500 mg twice a day given with meals. Dosage increases should be made in increments of 500 mg weekly up to a total of 2000 mg per day (one DIAPHAGE 1000 tablet twice daily). Dosage of DIAPHAGE 1000 must be individualized on the basis of both effectiveness and tolerance and should be started at a low dose with gradual dose escalation. The maximum daily dose is 3000 mg.

SIDE EFFECTS:
Transient gastrointestinal symptoms (diarrhea, nausea, flatulence and anorexia) may occur and usually resolve spontaneously during continued treatment. These symptoms can often be avoided by gradual dose escalation and by taking the tablets with food.

CONTRAINDICATIONS:
Hypersensitivity to metformin HCI, congestive heart failure, renal failure, diabetic ketoacidosis and hepatic diseases,

pregnancy and lactation:
As with other oral antihyperglycemics, the drug is not used during pregnancy and lactation.

DRUG INTERACTIONS:
Nifedipine may enhance the absorption of metformin. Alcohol intake potentiates the effect of metformin on lactate metabolism. Furosemide increases the metformin plasma concentrations.

PRECAUTIONS:
As with other oral antihyperglycemics, it may be necessary to withhold DIAPHAGE 1000 and temporarily administer insulin in case of patients exposed to stress such as fever, trauma, infection or surgery. DIAPHAGE 1000 may be reinstituted after the acute episode is resolved.
